Healthier Living with Chronic Disease, a Self-Management Program –

Do you have a chronic condition? CDSMP is to help people improve their health behaviors though exercise, cognitive symptom management, coping and communication to help improve their health status. In the workshop participants can share common concerns and problems to help manage their lives and emotions to become better self-managers. The workshop takes place once a week for a total of 6 weeks meeting for 2.5 hours.
