Since 2006, the California Departments of Aging and Public Health have been working together to support and increase statewide access to evidence-based programs for adults with chronic health conditions and disabilities. While both agencies have made progress, there are still many people in California who don’t know about or have access to evidence-based programs.
A Statewide Falls Coalition
In 2015, with supportive funding from the Administration for Community Living, the Coalition embedded falls prevention programming as an integral part of the Coalition’s mission. With this funding, the Coalition has made significant progress in developing a foundational infrastructure to implement and sustain falls prevention programming within the aging network, and has become a Statewide Falls Coalition with over 50 member organizations representing over 20 counties in California. By working with local partners, our mission is to:
- Increase access and availability of evidence-based health promotion programs in more communities,
- Raise awareness of the importance of evidence-based programs in helping people who are managing a fear or falling or who are managing one or more chronic conditions
- Decrease hospital utilization for falls by increasing falls prevention programming in communities
